Don’t be fooled by the main entrance! The lack of cleanliness, no blankets or thin ones, sometimes no sheets on the beds, shower rooms are a hazard, water is always cold, toilets clogged, poor standards, empty infection bins, and the list goes on. Yes, this is a business and of course you have to make money but how about actually CARING about the PEOPLE!

My grandmother has been staying at Diablo Valley, and the improvements are incredible. My grandmother has been living at Diablo Valley for a while now, and I’ve been really impressed with how well she’s treated here. The staff is attentive, patient, and genuinely cares about the residents, you can feel it every time you walk in. They recently started renovating the facility, and the updates are honestly top of the line. The new lobby looks fantastic, modern, clean, and way more welcoming. It’s clear they’re investing in making the environment better for both residents and families, and it shows. Seeing the effort they’re putting into the place gives me a lot of peace of mind. I’m really grateful for the care my grandmother receives and excited to see the rest of the renovations as they continue.

Stopped by recently and was really impressed with the updates they’ve made. The new lobby renovation looks fantastic—clean, modern, and very welcoming. You can tell a lot of thought went into creating a comfortable space for residents and their families. It’s great to see a facility that’s investing in improvements and really taking pride in the environment they provide. The management team clearly cares about the quality of care and the overall experience for everyone who walks through the doors. If you’re looking for a skilled nursing facility that’s continuing to improve and invest in its community, this is definitely a place worth checking out. Excited to see the rest of the renovations as they continue!

You don’t get a sense that your loved one is in a good place. It feels like your loved one is left in an abyss. It’s depressing and dismal

After months of trying to get in contact with James the program director of this place, I decided to take a trip there. I was helped by a kind lady named Patty at the front desk, but she informed me James wasn’t available and would call me. The next day he did finally call and I had a very disappointing experience with James. He is discriminatory toward students who take online classes and will not allow you to apply for the CNA scholarship if you are in nursing school. When I called to explain that my school schedule would not affect my availability, he was outright rude. He repeatedly interrupted me and dismissed what I was saying. Instead of listening, he basically told me I had no chance. I’m not sure if this was based on bias or discrimination against students, but either way, it was unprofessional and discouraging especially considering that many CNAs pursue nursing as a career path. It’s disappointing to see someone in healthcare treat motivated students this way. I would not recommend this organization to anyone who values respect and professionalism.

was sent here by Kaiser to recover from CHF and to walk again.. all my regular CNA’s from morning to pm to NOC were all perfect examples of good workers/ people. Johnrey morning, Carol/Caroline PM and Rosie NOC/Graveyard they are good examples of people who care and are professionals at what they do. Shoutout to Catherine and Meagan too who are morning CNA’s. There was also a registry nurse named Sharmaine who worked mornings and pm at times she was also a good CNA. Nurse Mika is really great at what she does and is very nice nd genuine, also nurse Cathleen who was basically my nurse case manager was very helpful and stayed in touch with my father. RJ is a very good social worker and helped me a lot as well. And the nurses who always made sure | was given meds on time like Ralph(AM), Gurdeep(PM) & Juan(NOC). There was another in the PM too but I forgot the name. The PT’s who regular saw and helped me were Alex and Rene, also OT was mostly Johnathan, they were all good therapists and very helpful. Overall my little over a month stay here was good.. there was also a very cute nurse l’d see roaming around sometimes named Kimmy, made my day every time I saw her.

Cna of my experience, it’s Manju Janelle, Patrick Hillwood, dessiia Elsa and the angry CNA Mr. Elliot I love these guys thank you so much. I appreciate your service. I like to say thank you to the wound care Kimberly Layla Trent Crazy Zellel and the best Remington Thanks so much my only concern is the kitchen using frozen canned vegetables. This is unhealthy and taste. Horrible you can use for vegetables. It’s much better. Please talk to the kitchen.

RJ social worker, Dr on staff, and nursing team all on point. Extremely friendly and helpful. They make the best of a rough situation for the patient. Social Services made great arrangements and coordinated in home care. Doctor arguably better than primary care physician and absolutely took the time to answer questions and check on my dad’s mental and physical readiness to go home. I have seen several of these places and while not fancy – it is safe and well attended.

My uncle is currently admitted to the facility. I will say everything has been amazing for the most part and everyone has been attending to him very well. The nurses are ok, but his CNA-David- has been attentive and taking great care of my uncle, and my uncle has nothing but nice things to say about him. I was hesitant to pick this facility due to the reviews left on here prior, but this facility has surpassed my expectations.

I had a great experience at this post-acute care facility. The therapy team was amazing—very professional, patient, and truly dedicated to helping residents improve and reach their goals. The AM nursing staff and treatment nurses were attentive, knowledgeable, and compassionate, always making sure care was done properly and on time. The AM CNAs were hardworking, kind, and treated residents with respect and dignity. You can really see the teamwork and genuine care here. I would definitely recommend this facility to anyone looking for quality post-acute care.

This place looks like one flew over the cuckoo’s nest. It’s a run down filthy place and it smells. My mother was dropped in a room and left there to freeze to death. Nobody checked on her and I was on my way to be with her. When I entered the building I could hear someone crying and yelling for help and to see that it was my mother I just flipped out. My mother was released from the hospital after being diagnosed with AFib and she couldn’t walk or even move her body due to Parkinson’s. She has been in a SNF before and the rehab was great and she was tended to but that was when she was able to function better than she is now. My mother needs care and this place doesn’t have that. It’s noisy all night long with people yelling and wandering the halls and TV’s blaring and staff talking loud amongst them selves that it seems like rehabilitation is impossible. The food is horrible, the communication is horrible and it’s hard to understand what people are saying since they have heavy accents and are wearing masks. My mother has done nothing but cry hear and it hasn’t even been 24 hours since arriving. The social worker here is helping me find another place for her because this place is horrible. I can’t believe Kaiser would contract with this facility. Also it’s not locked at night people can come and go and it doesn’t feel safe. This place should be investigated or shut down. It’s sad that you pay for insurance all your life and when you need quality care as a senior you just won’t get it. My mom is going backwards instead of rehabilitating. Thanks Kaiser for using such a shady rehab! P.S. The social workers name is RJ who is wonderful and the physical therapist is Sandy who is also wonderful so that needed to be said.

This place is absolutely horrible and feels like a real life death sentence. Whoever owns and operates this facility should be ashamed. It is painfully obvious that their only concern is how to squeeze more money out of insurance companies, while the well being, dignity, and safety of patients are completely ignored. If you truly care about your loved ones, do not send them here. Everything about this place screams neglect and hopelessness. The atmosphere alone feels heavy, cold, and unsafe. The staff often appear irritated, unapproachable, and as if caring for patients is a burden rather than their responsibility. On top of that, this is not a secure facility at all. I was able to come and go freely without ever signing in or being questioned, which is alarming and unacceptable for a healthcare environment. Patient safety is clearly not a priority. This facility should be investigated and shut down immediately. No one deserves to be treated this way, especially during such a vulnerable time in their life. MY GOAL IS TO GET PLACES LIKE THIS SHUTDOWN FOR GOOD 😊

I’m Shirley Yen, and although I haven’t lived here for very long, I’ve felt immense warmth. R.J. is the Social Coordinator, always thinking of the patients. Christian Cardenas, the Rehab Assistant, is very well-organized. Sonny Woodard, the Nurse, is dedicated and hardworking. Tran Nguyen, the Wound Nurse, often goes hungry to complete her tasks. Jean Capulong, the CNA, is always the first to help out.

They leave patients wet for hours and cnas as arerude and rough with patients. Food horrible and very noisy all night.over medicate ppl

my dad was sent here from kaiser after a mild stroke and care has been terrible at best!! gets a shower once a week if lucky,supposed to be twice a week, food is abosolute crap. they dont care for patients came in no wounds now has bruises on arm 2 bed sores ans a leg wound (pressure sores) they tested him for covid with no signs of covid because a speech therapist said he was congested but it is because they are thickening all his drinks. cant ever get a hold of anybody and force tests on people…after we put no vac,tests or meds until we notify us first, they dont read or pay attn the speech therapist is the worst she interupts all his meals so his food gets cold its bad enough hot. they dont rotate him in bed at all..pt and otc is decent. rooms somewhat clean. tv terrible,parking is the worst. LOOK FOR BETTER PLACES THIS PLACE IS TERRIBLE!!!! UPDATE 08/22/25 GOT DISCHARGED WITH BED SORES NOT READY FOR RELEASE THERAPY WAS PATHETIC. WE HAVE REPORTED THEM TO MEDICARE FOR FRAUD AND ELDER ABUSE. FILED COMPLAINT WITH STATE AND JAACO FOR BAD CARE, ABUSE AND INCOMPETENCE. HE LOST 28 POUNDS IN THE PLACE BECAUSE OF HORRIBLE FOOD! AND CARE

Read about how they gave a cup of cleaning solution to a patient and wouldn’t call 911 for 2 hours. Only after patient borrowed roommates phone and called 911 himself did they take any action. There’s complaints about neglect and crusty diapers.

They are one of the best in the Bay Area, very professional, tentative, and has a great staff. I’ve dealt with many facilities, this is the best I’ve ever been to.

Primarily worked with RJ in communications with DVPA. He is a busy man. Once I figured out who to talk with regarding my mom’s care, it went well (RJ works very well with long distance communication via text). They have great experience working people through the confusion of post surgery and therapy getting patients back on their feet. Parking is tight but workable. The facility is clean and there are large areas to visit outside of the patients rooms.

This has got to be one of the most unkempt, filthy, sloppy and horribly run nursing homes in Northern California. The place stinks inside, the food is terrible, the visiting areas are untidy and dirty with not enough chairs, they are understaffed so the patients receive little attention and most of the ones who work there show little concern. Items are stolen from the patients with no explanation, and the place as a whole is just totally depressing. I know first hand sll this to be true because my mother unfortunately is a patient here and I’ve seen some of 5hes3 things myself. Yes, if I was wealthy I’d place my mom in a much better nursing home, but I just can’t afford it and she’s dependent on Medicare/Kaiser. All other possible better places are full and she’s on a waiting list. This place is horrible and its a real shame how the state and the staff at this dump regard and treat the elderly in need of assistance. Go by and see for yourself.

Because this facility is in Concord I expected a high level of care for my mom. She arrived in the evening to an empty room that apparently had not been used recently, evident by the clock on the wall that hasn’t been changed to daylight savings time. There were no chairs in the room and they did not check to ensure that the air mattress on the bed frame was full, ready for someone to sleep on. Yes I said air mattress. They told me that they do not have regular bed or hospital mattresses on their beds. Her’s deflated which means she had to sleep basically on the hard frame. To try to get comfort, she put pillows underneath herself. The next morning they didn’t provide her towels to wash her face and she had to sit on the garbage can to eat her breakfast because they didn’t provide a chair for her to sit on. She finally asked for a chair because she refused to get back in the bed and lay on that hard mattress frame. That was around 9am. I arrived at approximately 10am, asked for a chair to sit on and pointed these things out to them. I was told they were having maintenance to fix her bed. It is now 11:50 and they still have not arrived to correct the issue. Mom is drowsy and wants to sleep but cannot get back in bed until they do so. I’m very disappointed in my decision to place her here versus the facility in Antioch thinking this would be a better place for her. I hate to say I was mistaken.

If I could give them a zero I would. My friend had a stroke with dominant hand sided hemiparesis. He is supposed to be getting physical therapy 3 times a week / why not 5 and.. they won’t transfer him they want to use his 90 days of Medicare. I saw his physical therapy video taped and it consisted of a what appears to be a non licensed, ancillary person RNA or CNA, well someone in scrubs throwing a large beach ball type object between him and another client. He was taken from his room for a total of 15 minutes. Did a real PT/OT generate orders based MD orders to eval and treat? I think not. This is a 190 bed circus act. The place needs investigation by the Ombudsman, DHCS and Medicare for fraudulent billing. Oh, by the way they are billing him an additional 4500 a month in addition to his Medicare coverage. It took a month to get a phone in his room. The food is not palatable. He has lost 40 pounds. He remained in bed and entire day I visited, they were short staffed. They would not get him up the entire day with 2 of us present and after multiple requests. I am an RN and it is notwmeworthy Only CNAs do direct patient care. Wait for your CNA to get you to the toilet or use the diaper we have taped on you. Request bedside commode multiple times and the urinal is stuffed in a drawer at the bedside and out of reach. This is post acute care rehab by this facility. Social services please arrange an immediate transfer for this human being you are falsely imprisoning in a bed all day while fraudulently billing Medicare. Transfer to JM post acute care rehab stat. Do your due diligence now because the care you are providing is not billable it is even marginally what is under the description of custodial care. Shame on you.

I’m staying here and I can confirm the bad comments, ( like the rudeness of many people, the hour waiting for someone changed your diaper) the nurses and CNA aren’t good and most are very rude, but also two of them are really nice and quick ant two bad I can’t said just two , the food well is nothing wrong is hospital food what we can expect, but I just wish with all my heart that the rooms can look like that, I a smaller room that is divided by a curtain so share with other people so you just got for your self no more than a two meters wide room. Wish I can go to a better place ? YES Medical can find me a new place? I don’t think so. I guess I have to resist until I can walk again and deal with everything 😞 Good luck and patience if you ever come here.
